Established in 1995, LifeLinks is contracted by the Disability Support Services within the Ministry of Social Development to facilitate Disability Support Services for people aged between 0-65 years who have an intellectual, physical, sensory or neurological disability (or a combination of these) and their families/ whānau/aiga/care givers. Our services are provided to the disability community and their families/whānau who reside in Canterbury, South Canterbury and the West Coast.
Within our management structure we have people with both physical and sensory disabilities. Also over the years we have been fortunate to employ people with disabilities and parents of children with disabilities within our Lifelinks team.
Each year LifeLinks partners with nearly 10,000 disabled people and their whānau to develop outcomes-focused personalised plans and facilitate their access to a tailored range of natural, universal and Disability Support Services that contribute to their achievement of the outcomes they seek for their lives. There is no cost to you for receiving LifeLinks outcome planning and outcome facilitation services.

LifeLinks is a values-led organisation supporting disabled people (tangata whaikaha), their whānau, carers, and communities across Canterbury, South Canterbury and the West Coast.

The Opportunity
This role offers meaningful, focused work where you will be well supported and able to see the impact of your work each week. You’ll help people navigate change with clarity, dignity, and respect.
This is a 12-month fixed-term project role created to support a significant period of system change within disability support services.
You will be part of a dedicated project team supporting people connected with LifeLinks through a mix of assessments and reassessments, outcome planning, and coordination activity, ensuring plans and supports reflect people’s current lives, needs, and aspirations.
Experience in disability, health, social services, coordination, administration, or assessment work is highly valued. We also welcome applications from graduates and early‑career professionals with relevant qualifications and a strong interest in disability support.
The Role:
As a Project Facilitator, you will work alongside disabled people and their whānau to gather information, support planning and coordination conversations, and prepare high-quality documentation that supports timely, consistent, and accurate decision-making.
Depending on project needs, your work may include a combination of reassessment activity, outcome planning, and coordination support.
